Four wrinkle relaxers on one shelf, and how one of them gets chosen

The four do not share an onset curve and do not share a duration. This is the selection procedure: the area, the calendar, whether someone is new or established, and what the two-week follow-up catches.

There are four wrinkle relaxers on the shelf here. Dysport, Daxxify, Jeuveau and Xeomin, each dosed by area, each quoted before anything is booked. Four manufacturers, and most practices near here carry one.

The question that arrives with that list is almost always which of the four is better. That question does not have an answer. Which one gets used is a decision about the area, the calendar, and what happened the last time.

How long does Daxxify take to start working?

Generally a day or two ahead of the others, and the gap is smaller than the marketing around it suggests. Something starts happening with all four at roughly day two to four, most of it has arrived by day ten, and the honest answer sits at two weeks.

Onset is the part people ask about. Duration is the part that decides more. Xeomin, Dysport and Jeuveau generally run three to four months for most people. Daxxify generally runs longer, and that is what the difference in price is for. For some people that trade is the right one and for others it is not, which is a calendar and cost question rather than a clinical one.

Does the area being treated change which one is used?

Yes, and it is the largest single input into the decision. Dysport generally spreads a little more than the others, which I tend to like across a forehead, where the point is an even result over a wide flat area. That same spread is a reason for caution near an eyebrow or around a mouth, where a couple of millimeters in the wrong direction is the difference between a result and a problem.

Xeomin carries no complexing proteins around the molecule, and it is generally where I start when precision matters more than reach. Daxxify is peptide-stabilized, and the duration is the reason to reach for it rather than the area.

Does it matter whether someone has had this done before?

It changes the first choice quite a lot. For someone new to neurotoxin I generally start with a shorter-acting product and one area. If the result is not right, three to four months is an easier thing to sit with than six, and a first treatment is partly an experiment about how one person metabolizes one molecule.

For an established patient the chart does the work. Which product, how many areas, how it looked at two weeks, and whether it had gone by month three. Those four facts point at the next choice more reliably than any brand comparison does, which is also why the chart is worth keeping when somebody is treated in two places in a year.

What does the two-week follow-up catch?

It catches the difference between what a product was on its way to doing and what it did. Some of it I can adjust at that visit. Some of it I cannot, and that wears off on its own schedule. Either way it is a much better conversation at two weeks than at four months, and the visit is part of the treatment price rather than an addition to it.

When is a different practice the better answer?

Two situations, and both happen in an ordinary month.

  • A request for a brand I do not stock. I would rather someone went to a practice that carries it than had me talk them into a substitute. A substitution nobody wanted is a poor way to begin a two-week wait.
  • Anyone inside about four weeks of a wedding, a photograph or a flight. Neurotoxin is predictable enough that it is usually fine, and "usually" is not a word worth attaching to a wedding. Later is a better answer than the week of.

What do the prices actually tell anyone?

They tell anyone what a treatment costs here, and nothing else. Your quote is fixed at the consultation and it does not move between one person and the next. What the gap between them does not do is rank the products. It reflects what the molecule is and how long it generally lasts.

Pricing by area rather than by unit is deliberate for the same reason. When the number moves with the count, the person deciding the count is the person the number belongs to. I would rather that was not a question anyone has to hold in their head while sitting in the chair, which is also most of what the reasoning page is about.

The thing worth tracking is not which brand a friend was happy with. It is month three. Two appointments, with the treatment date and the date the result faded written down beside each, produce better information about which of these suits one particular face than any comparison written by somebody who stocks only one of them.

Questions people ask about this

Is one of the four stronger than the others?

They behave differently rather than one being stronger. Units are defined by each manufacturer's own assay, so the numbers are not comparable across brands, which is why everything here is priced by area rather than by unit.

Can someone switch from one to another?

Generally yes, and plenty of people do. It is better done at a first appointment of the season than immediately before an event, so the two-week follow-up has somewhere to land.
